From The Register-Guard (Eugene, Oregon):
Aformer Springfield resident, convicted of murdering his mother in her Eugene home in 2004, escaped from the supervision of the Psychiatric Security Review Board.
The review board sent out a notice Thursday afternoon, which was shared by Eugene police, asking for the public’s help to find Thaddeus Ziemlak, 36.
Ziemalk, who was last seen on the morning of Jan. 5, when he was leaving his residential facility in Pendleton on a planned community outing. He was due back at his residence at 2:30 p.m., but did not return.
Ziemlak is a white male, 5-foot-10-inches tall, 170 pounds with sandy brown hair and green eyes. He has many acne scars, causing his face to look pockmarked. Ziemlak was last seen wearing a black, zip-up sweatshirt with a hood, blue jeans, black tennis shoes and a black, beanie-style hat. He wears wire-rimmed glasses and normally walks with his head down. He also was carrying a black backpack, according to the notice.
When last seen, Ziemlak appeared psychiatrically stable, but is considered a danger to others, the notice reads. He has a history of drug abuse, which can exacerbate his symptoms. The Psychiatric Security Review Board urges anyone who sees Ziemlak to call police immediately, but not to approach him.

According to The Register-Guard archives, Zeimlak was arrested by Eugene police in February 2004 after his mother was shot twice in the head at her home. Minutes before she was found by police, Joyce Zeimlak had called 911 to report her son was armed with a rifle and threatening to kill her, newspaper archives show. Zeimlak, 20 at the time, was charged with murder but was found guilty except for insanity in September of that year, court records show. He was sentenced to the supervision of the Oregon Psychiatric Review Board.
Joyce Ziemlak had worked at Lane Transit District for 10 years and was a senior human resources analyst.
Since that time, Zeimlak has twice been charged with escape from the psychiatric review board’s custody, court records show, in November 2006 and November 2009. In 2006, newspaper archives show Ziemlak was on an approved pass when he ran from a hospital staff member about 4:45 p.m. while visiting a sporting goods store in Salem. He was later found in Wenatchee, Wash. The details of the 2009 escape were not known.

Thoughts? Local online news reports had Ziemlak as still at large at least as of yesterday (January 24th, 2020) and also I could not find a specific number for tips but rather most articles just say to call 911 immediately if you spot Ziemlak.
Also, someone needs to explain to me how this guy escaped not once but THREE times. I demand immediate eye doctor tests for everyone at this facility.
